Waiver and Release of Liability
In consideration for being permitted to participate as a volunteer in activities organized by the Burlington Lions Club (“the Club”), I agree to the following:
• I understand that volunteering may involve physical activity, work near vehicles or equipment, and exposure to weather and crowd conditions, and that some risk of injury or property damage exists in any such activity.
• I will follow the direction of my supervising Lions member and all posted safety instructions, and will immediately report any injury, hazard, or incident.
• I voluntarily assume the ordinary risks associated with my volunteer role and release, discharge, and hold harmless the Burlington Lions Club, its members, officers, and directors, and Lions Clubs International, from any claim, liability, loss, or damage arising from my participation, except where caused by the gross negligence or wilful misconduct of the Club or its members.
• I understand the Club’s general liability insurance covers claims by third parties arising from my volunteer activity, and that it does not provide medical or accident coverage for my own injuries. I am responsible for my own health coverage unless the Club confirms separate volunteer accident insurance applies to this event
I consent to receive basic first aid if needed during my volunteer shift.
I have read this waiver, understand its terms, and sign it voluntarily.
